Each interview type (e.g., Phone Screen, Onsite, or Video Interview) in Jobvite ATS can have a unique evaluation form associated with it on a per-requisition basis. Even if multiple interview types use the same-named evaluation form, the actual questions within that form are customized and unique to the specific interview step they are configured under.
This means that an evaluation form configured for the Onsite or Video Interview will not automatically share its questions with the same evaluation form configured for the Phone Screen.

How to Ensure the Correct Evaluation Form is Sent
To ensure the interviewer receives the correct evaluation form with the right set of questions, the evaluation must be customized and updated under each individual interview type.
Navigate to your user menu > Admin.
Click ATS.
Click View Interview Types.
Choose the specific interview type you are scheduling (e.g., Phone Screen).
Locate the specific evaluation form selected on the requisition for this interview type. You must update and verify the content of the evaluation form under that specific interview type.
You will need to update the evaluation under each interview with the questions that the evaluation should ask at that specific interview step.
Known Limitation
There is no sharing between evaluations. Evaluation questions are not shared between evaluations and interviews. Each evaluation is customized at the specific interview step, and they don't copy from other evaluations in the same stage.

Q. Why did the wrong form go out?
A. If you scheduled one interview type (e.g., Phone Screen) but the interviewer received the evaluation form for a different interview type (e.g., Onsite or Video Interview), it indicates that the evaluation form selected on the requisition for the scheduled interview step is either empty or incorrectly configured.
